Do you have an idea that could improve the connectivity of care for patients as they leave the hospital? Help spread the word, or submit your idea, to the Janssen Connected Care Challenge for an opportunity to benefit from the 250,000 dollars in awards to develop your idea into a business.
The Janssen Connected Care Challenge is now accepting submissions of concepts that improve the experience and outcomes of patients as they transition from hospital to home care. Successful solutions will:
- Illustrate the potential to improve health and post-hospital care.
- Demonstrate the ability to lower the cost of unnecessary readmissions and physician services.
- Utilize technology that patients and physicians can easily access or acquire.
- Show the capacity to scale with minimal cost.
Submissions will be judged by a panel of experts in the areas of care transitions, mobile health, care coordination and healthcare business.
