The Janssen Connected Care Challenge is now accepting submissions of concepts that improve the experience and outcomes of patients as they transition from hospital to home care. Successful solutions will:
- Illustrate the potential to improve health and post-hospital care.
- Demonstrate the ability to lower the cost of unnecessary readmissions and physician services.
- Utilize technology that patients and physicians can easily access or acquire.
- Show the capacity to scale with minimal cost.
Submissions will be judged by a panel of experts in the areas of care transitions, mobile health, care coordination and healthcare business.
